Steps: 
1) Check for updates and any manual code changes you need to make. Usually there are none.
2) Take a screen shot of the update page so you know what versions you started with (in case you need to restore later)
3) Backup the database. Save this! (use backup migrate)
4) Download any core or contributed module updates.
5) Put the Site in maintenance mode (off line). Its best to do this in the wee hours of the morning.
6) Upload the core system EXCEPT for the sites folder, .htacess file and any other files you have customized.
7) Log in as the primary user and run the update.php script.
8) Make note of any errors. If you can't fix them, undo what has been done. Usually there are none.
9) Upload 5 contributed modules.
10) Run the update.php script.
11) Repeat steps 8 and 9 until all modules are updated.
12) Check the site status for other problems
13) Check the update page and make sure you got them all.
14) Take the site out of maintenance mode (on line).

Had to update the core twice for some reason? But - no errors or problems.
